The thing where you look up and an hour's gone.

The thing you postpone, avoid, or wince at when it comes up on the calendar.

What's piling up?

Tabs you can't close, a folder you keep not opening, paperwork in a corner, an inbox you've stopped looking at. Anywhere avoidance has accumulated.

When you can't sleep, what's the work thought?

Or: what's the first work thought when you wake up. Whichever's louder.

Where does the pain mostly live?

Pick as many as feel true. There's room to add your own.

A month from now, what would "this got better" actually look like?

Doesn't have to be realistic. What would you notice on a Tuesday?

Anything else you'd want me to know before we talk?

A doc, a screenshot, a story, a worry. Or just write "I'll tell you." Whatever you'd rather not write in a form is also a useful thing to know exists.
