This is the intake I read before we talk. It is meant to find the parts of your work that are taking more attention, time, or dread than they should.
A sentence or two, the way you'd say it to a friend at a barbecue. Not your title.
The thing where you look up and an hour's gone.
The thing you postpone, avoid, or wince at when it comes up on the calendar.
Tabs you can't close, a folder you keep not opening, paperwork in a corner, an inbox you've stopped looking at. Anywhere avoidance has accumulated.
Or: what's the first work thought when you wake up. Whichever's louder.
Pick as many as feel true. There's room to add your own.
Doesn't have to be realistic. What would you notice on a Tuesday?
A doc, a screenshot, a story, a worry. Or just write "I'll tell you." Whatever you'd rather not write in a form is also a useful thing to know exists.
I'll read what you sent and reach out to find a time to meet!
